Does a warning have to mention dismissal?
Usually you should say further action may follow under your disciplinary procedure, which this template does. Do not threaten more than you intend to do.
Should the employee sign it?
Ask them to sign to acknowledge receipt, not agreement. Keep a copy in the personnel file either way.
